About the Birth Control Pill

I have a question regarding the BCP regarding it's effectiveness when used PROPERLY and would like to hear your thoughts and experiences. I have heard so many stories in the past of women becoming pregnant while using the BCP but although I am no expert in any way, I always find myself questioning why they did get pregnant. I usually figure that that person was not taking the pill the way it should be taken (ie; same time EVERY day etc.) because acccording to our family doctor, she says that my wife and I are fine to not use condoms provided my wife takes the pill properly which she is excellent about. My wife just finished nursing our 1 year old and she has since gone back on the pill. We are planning to stop using condoms soon and just use the pill for BC based on our doctors instructions.
So, how many of you have, or know someone who has, become pregnant while using the BCP and KNOW that you/they were following its directions properly?

but my mom also has 6 kids, fertility kinda runs in the family.
and go easy on wikipedia links, everythign is user submitted and i've found some nastily false information on there. take wiki with a grain of salt

I guess it depends on alot of variables though.

Perfect-use failure rate is 0.3% or 99.7% effectiveness.
