Be delicate when you go in.
Try to look at her and feel her, to see if she's being hurt, if she's enjoying it, and how she likes it better.
Try not to cum before she even starts to like it.
Keep in mind that after you'll have cum you probably won't be able to keep going. It might even hurt a bit if you try.
And good lick! (that0's a good way to warm her up...)

In regards to making sure she is ok, do so by paying attention to her body language, and expressions like athmyrrdin recommended. Do not keep asking her if she is ok, that will only add tension.
The reason why so many women hurt on their first time is because they are so tense. The better you can relax her before, and durring, and after the better the experience will be for both of you.
I've always suggested watching some porn. You shouldn't try to mimick what they do, but it should give you more of a comfort level with it. You just might pick up a tip or two from watching them as well.

always start slow, and get things in the mood. kissing, making out, using your hands (but dont rush down there first).
when you do it, dont just do the same motions repeatedly, that can get boring and might make u go too fast, try changing things up a bit.
and i typically stop after shooting my load, and pull out. but i always make sure the girl cums first.
